I thought this story was wickedly endearing and... Lovely.
"It even hurt to cry. So she tried not to care about that, either."
Those simple sentences were unbelievably powerful in this one-shot. Eloquent and poetic, they captured Harley's pain and crammed it into a nutshell.
BEAUTIFUL.
Anyway, I'm running off to comment on all of your other stuff that I have read and didn't:
A) have the time to review (I read mostly at work)
B) Forgot. (I try not to, and I'm usually pretty good about it. But you just have so MUCH to read...)

Nice job as always. I agree that this is in character for his reactions to take a very long time to go from ignoring her illness to getting irritated by her illness to finally dealing with her illness. Smacking her around and then dumping her body outside of Arkham. How romantic!
As for suggestions for the fluff piece, if Harley were to become sick or injured and Joker were to take any interest in caring for her, the majority of the nursing would probably have to be carried out by somebody else: a doctor in their hostage, one of the more educated goons, or maybe one of the villain doctors, agreeing to help as part of some larger arrangement. Joker could have fun playing doctor for a while, maybe on and off, but he's not the type to keep playing when the game gets boring. There'd have to be somebody to serve as backup. Otherwise he'd probably just "treat" her for an hour and then leave her alone to starve in bed for a week. I don't know if you can make that into a sexy scenario. But hey, you've surprised me before! Go with whatever you think is best!

I probably shouldn't write reviews when I am half asleep, but my personal radar picked up a blip (your story) and so I just had to read it, and thus leave a review...well, here goes nothing then....
I love Dr. Leland's remark of "I don't think I could handle the concept right now and Harley needs antibiotics."
